Evidence-Based Interventions

Dr Ravi offers a comprehensive suite of interventional pain procedures including spinal injections, radiofrequency ablation, nerve blocks, regenerative medicine, and multimodal pain management, each performed with precision and care at Cygnet Clinic, Fremantle.

01

Epidural Steroid Injections

A cornerstone of interventional pain medicine for spinal pain and radiculopathy.

Lower Back PainSciaticaSpinal StenosisDisc HerniationNeck Pain

Preparation

Patients may be asked to cease blood-thinning medications prior to the procedure. Fasting is usually not required. The procedure is performed as a day case.

The Procedure

Using real-time fluoroscopic (X-ray) guidance, Dr Ravi places a fine needle into the epidural space and delivers a combination of local anaesthetic and anti-inflammatory corticosteroid directly to the inflamed nerve root.

Recovery & Results

Most patients can return home within 1 to 2 hours. Temporary numbness or leg weakness may occur. Significant pain relief typically develops over 3 to 7 days.

02

Radiofrequency Ablation (RFA)

A minimally invasive technique providing long-lasting relief for facet joint and sacroiliac joint pain.

Facet Joint PainSacroiliac Joint PainCervical Facet PainKnee Osteoarthritis

Preparation

Diagnostic medial branch blocks are performed first to confirm the pain source. If successful, RFA is scheduled as a follow-up procedure.

The Procedure

Under fluoroscopic guidance, a specialised radiofrequency needle is positioned adjacent to the targeted nerve. Controlled heat is applied to temporarily disrupt the nerve's ability to transmit pain signals.

Recovery & Results

Patients may experience mild soreness at the needle sites for 1 to 2 weeks. Pain relief typically develops over 2 to 4 weeks and can last 6 to 12 months or longer.

03

Nerve Blocks

Targeted injections of local anaesthetic near specific nerves for both diagnosis and treatment.

Occipital NeuralgiaHeadachesFacial PainNeuropathic PainCRPS

Preparation

Minimal preparation required. Patients should inform the clinic of all current medications and any allergies.

The Procedure

Using ultrasound or fluoroscopic guidance, local anaesthetic is delivered precisely around the target nerve. The type of block is selected based on the pain condition and anatomy.

Recovery & Results

Immediate temporary numbness is expected. Diagnostic blocks help confirm the pain generator. Therapeutic blocks may provide relief lasting weeks to months.

04

Joint Injections

Image-guided delivery of medication directly into painful joints for rapid and targeted relief.

Hip ArthritisKnee ArthritisShoulder PainSacroiliac Dysfunction

Preparation

No fasting required. Patients should disclose any prosthetic joints or joint replacement hardware at the time of booking.

The Procedure

Using ultrasound or fluoroscopic guidance, a fine needle is placed into the joint space. Anti-inflammatory medication, local anaesthetic, or hyaluronic acid may be injected depending on the condition.

Recovery & Results

Patients can usually resume normal activities the same day. Initial relief from local anaesthetic is immediate, with anti-inflammatory benefits developing over the following days.

05

Spinal Cord Stimulation

An advanced neuromodulation therapy for select patients with chronic, refractory pain conditions.

Failed Back Surgery SyndromeCRPSPeripheral NeuropathyRefractory Chronic Pain

Preparation

A comprehensive assessment and multidisciplinary team review is conducted before proceeding. A trial stimulation is performed first.

The Procedure

Thin electrode leads are placed in the epidural space under fluoroscopic guidance. These deliver mild electrical pulses that modulate pain signals before they reach the brain. A trial period allows patients to assess effectiveness before permanent implantation.

Recovery & Results

The trial typically lasts 5 to 7 days. If successful (at least 50 per cent pain reduction), permanent implantation is scheduled. Most patients experience significant improvement in pain and function.

06

Regenerative Medicine

Emerging therapies that harness the body's natural healing capacity to repair damaged tissues and reduce pain.

TendinopathyOsteoarthritisLigament InjuriesChronic Joint Pain

Preparation

Comprehensive assessment to determine suitability. Review of medications and any contraindications. The procedure is explained in detail with informed consent obtained.

The Procedure

Regenerative treatments may include platelet-rich plasma (PRP) injections or prolotherapy. These involve injecting concentrated healing factors or mild irritant solutions into damaged tendons, ligaments, or joints to stimulate natural healing and tissue repair.

Recovery & Results

Some post-procedure soreness is normal for 3 to 7 days. Gradual improvement occurs over several weeks as the body's healing response develops. Activity modification may be advised during the recovery phase.

07

Multimodal Pain Management

A comprehensive, patient-centred approach combining multiple treatment modalities for optimal outcomes.

All Chronic Pain ConditionsFibromyalgiaChronic Widespread PainCancer Pain

Preparation

Thorough initial assessment including review of medical history, imaging, and psychological factors. Collaboration with the multidisciplinary team at Cygnet Clinic.

The Procedure

Dr Ravi develops an individualised management plan that may include medication optimisation, interventional procedures, physical therapy, cognitive behavioural therapy (CBT), chronic pain education, exercise programs, psychological support, and lifestyle modifications.

Recovery & Results

Ongoing follow-up and plan adjustments ensure continued progress. The goal is sustained improvement in pain, function, and quality of life through coordinated multidisciplinary care.
